JOB DESCRIPTION:

The goal is to achieve and maintain housing stability. Case Manager will be responsible for responding to housing crisis and participants concerns, supporting participants to maintain their housing, assist them with their housing transition plan, and meet their person-centered service plan goals. Case Managers are also responsible to completed efficient housing searches and support landlord & neighbor relationship building. It’s essential to collaborate with one’s care team to meet their community objectives and goals. 

PRIMARY OBLIGATIONS:

  • Assessing Person-Served eligibility and determine their housing and service needs.
  • Assisting Person-Served with effective and efficient discoveries for available and suitable housing.
  • Develop a customized, Person-Centered housing plans with participants.
  • Provide support aiding in applying for benefits to increase financial independence, various skills abilities, access to essential community resources, and connecting with various providers.
  • Respond to Person-Served plan and requests as they arise and work to problem solve and deescalate situations.
  • Provide community-based Case Management that includes frequent face-to-face and phone contacts with each Person-Severed, with a caseload of approximately 5 and/or supporting one's abilities.
  • Communicate with care team members, community professionals, and other service providers to ensure that services for the Person-Served are properly coordinated and obtained.
  • Collaborating with government agencies, community organizations, and property owners.
  • Preparing and maintaining electronic health records, activity documentation, and reports to ensuring it is in accordance with the expectations for timely and thorough billing and in compliance with HIPPA and Data Privacy Practices.
  • Understanding and adhering to programs rules and restrictions.


PROGRAM EXPECTATIONS:

  • Ensuring Person-Served understands their rights and responsibilities as tenants and participants of other programs.
  • Transport of Person-Served in your personal, insured, vehicle for housing and sustaining housing purposes only.
  • Providing Person-Served with information and refer them to additional support programs or services.
  • Provide flexible services within a schedule that meets the needs of Person-Served.
  • Continuously exercises good judgment in handling Person-Served issues.
  • Represent the organization with a positive and professional image, at all times.
  • Provides and promotes unconditional positive regard to all Person-Served, regardless of race, creed, gender, religion, sexual orientation, disability, and economic status.
  • Meets performance criteria as outlined in annual performance evaluation.
  • Participate in staff meetings and meet regularly with program coordinator, manager and/or executive team.
  • Maintain current knowledge of housing and community requirements as they develop.
  • Performs other duties as part of a whole-"TEAM work, make the DREAM work".


EDUCATION AND SKILL QUALIFICATION:

  • Bachelors for RSC/MHM providers and high school diploma/GED for aides and HSS providers.
  • Minnesota current and valid driver's license, and an acceptable driving record.
  • Reliable Transportation with valid auto insurance.
  • The ability to work with integrity, independently.
  • Knowledge of housing resources, social service agencies and community services.
  • Experience with working in a diverse, continuum of services in a fast-paced, growing agency.
  • Good verbal and written communication, diverse interpersonal, and active listening skills.
  • Strong analytical, mediating, and negotiation skills.
  • Excellent organizational and case management skills.
  • Comply and retain a thorough working practice of agency policies, protocols, and procedures.
  • Leadership skills and the capability of working alone with very little supervision.
  • Support with innovation and tenacity towards providing creative solutions in supporting individuals in their community- safety, with a vision for having a holistic approach.
  • Ability to understand and operate Microsoft Office programs and software.
  • Ability to type 45 WPM.


Job Details:

  • WAGES: $21.00 -$24 per hour, DOQ.
  • Job Types: Full-time
  • Willingness to travel: 100% (Preferred).
  • Work Location: In person.

  • Case management: 1-3 years (Preferred).


License/Certification:

  • Minnesota Driver License and active insurance. (Required)
